What is the procedure to open DEMAT account in Axis Bank?

I now stay in Chennai in Tamil Nadu.But I grew up in Neyveli,another town in Tamil Nadu.I have PAN card.I have a driving license in which the address printed is my Neyveli address.I also have a ration card proof in which also my neyveli address is printed.Can i open a DEMAT account in Chennai in Axis bank?
